You may request an evaluation through the link below. If Dr. Clark is available at your requested time, you will receive an email and/or text message with a link to complete intake paperwork. Your appointment time will not be confirmed and held until the intake paperwork is completed and returned. If we have not received completed paperwork within one week of your appointment request, your requested time slot will be reopened for others to schedule. Once we have the completed intake paperwork, you will receive an email confirming your appointment date and time. If you have any questions, please contact our office at 254-262-6743 or jhclark@ctxneuropsychology.com.
