Every evaluation begins with an intake appointment. Choose the intake matching who it's for and how you'll pay.
Your appointment is confirmed once your intake paperwork is complete. You'll receive it by email right after booking (15–20 minutes). Please finish it within 3 days - we use your answers to prepare for your visit and send your initial written estimate. Your exact all-inclusive fee comes after intake, and testing is scheduled only once you've approved it. Unfinished paperwork may require rescheduling.
Legal, guardianship, or disability evaluations cannot be booked online - contact our office for a fee agreement.
