Every evaluation begins with an intake appointment.
Your appointment is confirmed once your intake paperwork is complete. You'll receive it by email right after booking (15–20 minutes). Occasionally, these emails go to spam. Please check your spam folder if you do not see an email with intake paperwork within 1 hour.
Please note: All digital intake forms and questionnaires must be completed at least one week (7 days) prior to your appointment. This allows us time to verify your insurance benefits. If forms are not complete, your appointment will be automatically canceled to open the slot for another patient.
Legal, guardianship, or disability evaluations cannot be booked online - contact our office for a fee agreement.
